Session Two: Completion + Clarity
Together, we’ll finalize your essential documents — including your Advance Directive, Power of Attorney, HIPAA release, and Dementia Directive. We’ll also prepare a clear plan for how you’ll share your decisions with those who need to know.
Session Three: Follow-Up Support
After you’ve shared your plan, we’ll meet again to talk through how it went. What landed? What still feels unsettled? You’ll get guidance on how to revisit the conversation if needed — and peace of mind that you’re not navigating it alone.
